Job Description
- Facilitate care plans and monitor budgets while providing case management for clients.
- Conduct regular check-ins to assess client goals and make necessary adjustments to plans.
- Approach sensitive topics, such as changes in care needs or funding, with compassion and a solutions-oriented mindset.
- Escalate concerns related to health or wellbeing through established clinical pathways.
- Oversee administrative tasks to ensure compliance with Aged Care Quality Standards.
- Build positive relationships with clients, their families, and service providers.
